Special Notice Regarding NGS Paper Vouchers

NGS sent an email today about the discontinuation of paper vouchers for new providers or providers that re-enroll (which Medicare normally requires annually). Most Medicare contractors follow NGS within months of NGS changes.

Attention Medicare Providers that National Government Services (NGSMedicare) announced that as of May 13, 2019, any new provider or any providers that change information with NGSMedicare, will no longer have the options of receiving paper vouchers.

If you are currently receiving paper vouchers, you should begin working with your clearinghouse to ensure that you will have no interruptions in receiving your vouchers. Your clearinghouse may have a registration process that needs to be completed prior to you being able to receive the 835’s.

Delays in receiving vouchers can affect your ability to post payments and properly bill a secondary insurance or patients.

If you bill a Medicare contractor other than NGS and receive a paper voucher, watch your vouchers to verify that your contractor is not in the process of making similar changes.

Good Coding Really Does Equal Good Patient and Pocket Outcomes

By Ted A. Arkfeld, DC, MS

Over the past few months, I have been contacted by different doctors around the state who were experiencing either denials from Medicare, BCBS, or needed some additional training on how to use ehrThomas in their office. When reviewing their files I noticed errors in the sequencing of their diagnosis codes. Incorrect diagnosis codes and improper linking of the codes to the CPT codes can cost you money. The most glaring issue that I come across is the incorrect Evaluation & Management Code selection. In one file that was reviewed the doctor did an amazing history, comprehensive examination, but used the 99212 Established Patient Code, where they should have used a 99214 code. A 99212 will pay $47.32, whereas the 99214 would pay $116.01. This is a $69 difference, now what if that occurs twice daily for five days. This equates into a loss of revenue for one week on just this one code of $690. Over a four-week time span, the office is losing $2,700.

Are You Passionate About Chiropractic or Going Through the Motions?

By Ted A. Arkfeld, DC, MS

First of all, I hope the subject line caught your attention and you are still reading this email. I have had the pleasure of visiting doctors and offices all across the nation, and I can tell within the first five minutes if the doctor and staff are on task, passionate, and successful.

Unfortunately, I have visited with way too many doctors who have lost their passion and zeal for practicing in today's healthcare. They put on the brave face and head out each day to the office to do the same thing they did yesterday. Oh, don't get me wrong, they may be making a good living, and have a relatively stress free practice, but they are not having fun.

The reason I can identify so easily with these doctors is that I was there three years ago. I had reached the 27-years in practice mark and was not passionate about my practice. That all changed one day as I sat in on a presentation by Patrick Porter, Ph.D., on brain stress. He was actually talking about patients who were sympathetic dominant, and how chiropractic care was the answer.

From Concussions to Medical Necessity

Dr. Arkfeld (Ted A. Arkfeld, DC, MS) held a great Concussion seminar here at Genius Solutions last week. Ted is amazing and he always brings extras... extra new gear to try and love, ways to improve and add to the offerings your office can give to improve the lives of your patients, and how to best improve your bottom line... often with gear you already have (plus I know how much he loves to make sure you are fed).  If you have never seen him live - I strongly recommend you take the opportunity when it is next given. You won't regret it. He has a love of chiropractic, chiropractors, and patients that you don't always get to see or experience. And his knowledge of coding and documentation can't be beaten.

Get Better at Coding, Get Paid Better

In case you haven't thought about it in a while... here is a little article he sent my way on medical necessity and his mini Clinical Genius course - in case you would like a refresher.
Medical Necessity is simply why we get paid for services rendered to a patient who has insurance. Most carriers follow Medicare's definition:

The patient must have a significant health problem in the form of a neuromusculoskeletal condition necessitating treatment, and the manipulative services rendered must have a direct therapeutic relationship to the patient's condition.

How do you support medical necessity?

eTHOMAS Update

Special Notice-Please Update your eTHOMAS system

  1. Additional changes have been made relating to Medicare insurance payment posting functions that affect both manual posting and autoposting. Some customers have noticed that small insurance balances were left on claims after payments have been posted. The additional changes we have implemented will properly write off those balances and handle newer Adjustment Codes we are receiving from Medicare at this time. Customers will notice that after the update has been completed, these balances are adjusted off by either an insurance credit (with the System Setting called inspostwithholdperline) or as a PARADJ without the setting.
  2. This update also includes enhancements that make the sending and receiving of information to and from Genius Solutions, Inc. and our Customers run even better.Previously, some Customers may have received errors when syncing information with Genius Solutions, Inc., or are not receiving our broadcast messages properly. When changes are necessary for your computer to complete those functions you will receive the following message:

When you see this message, you should choose Yes. This will allow you to sync contacts, add and expire providers, and receive broadcast messages without errors or interruptions.

Chiropractic Care For Concussions

Genius Solutions', MAC, & Clinical Genius Present:
Chiropractic Care For Concussions With Ted A. Arkfeld, DC, MS

From Dr. Arkfeld
Dear Doctor,
I am pleased to announce that in conjunction with Genius Solutions and 8-Hours of CE's through The Michigan Association of Chiropractors, we will have a Chiropractic Care for Concussion Seminar in Michigan.
Every year over 4-million of concussions are reported in the US alone. This number is actually low because untold numbers are never reported. The number one recommendation for these patients is to go home and rest, that it will take time.

Special Notice Regarding Medicare Autoposting

Some eTHOMAS Autoposting customers may experience issues with autoposting Medicare payment for dates of service 1/1/2019 and after. Medicare has changed some reason codes listed on the 835 payment reports for certain providers. Previously in 2018 this code was N701 and has now been replaced with reason code N807. Customers who receive this edit are receiving a MIPS reduction amount on their payment and this reduced payment amount will not be removed from your claim resulting in a balance still remaining on the claim after it is posted.

What you can do if you encounter this new edit:
  • Option 1.
Update your eTHOMAS software as soon as possible. The new version number will be 9.5.49 in the upper right hand corner of your main eTHOMAS screen. Even if you have updated already since January 29th, 2019 and are on version 9.5.49, please update again as this modification has just been added as of February 6, 2019.
  • Option 2. (if you do not update)
While autoposting, and if this small edit balance is showing up in the Balance column in the Loaded Check screen, click the Writeoff Balance check box to the right of this balance to remove the excess amount from the claim.
  • Option 3. (if you have already autoposted some payments or do not update)
You can go back to the Patient’s Claim File after autoposting and write off the balance on the claim manually.
We believe this may affect a small number of our customers, but please review your Medicare vouchers closely to see if you have this new edit, otherwise it will not affect you.

Thanks for being a Genius client and letting us serve you.

Sincerely,
— Genius Solutions, Inc.
